Online services Instructions
In an effort to improve your communication with us and to allow our providers to more efficiently evaluate your child's developmental needs, we offer secure web services. These services can be accessed from our website through the red "On-line Services" door. We offer "Patient Web Portal" and "Chadis" (a Pre-Visit Screening Tool). Through our "Patient Web Portal" you can review, update or enter your demographic, insurance, and family/social health information. You can also request an appointment, request a prescription refill, as our staff questions or pay your bill. Through our Pre-Visit Screening Tool (Chadis) you are able to complete necessary developmental screening forms from your home, at your convenience. These tools give our providers insight into your child's development and allow for earlier intervention when necessary.
Patient Web Portal Instructions
In order to create an account on our "Patient Web Portal" you will need to receive a portal invitation with a "Sign Up Here" link. You will be required to fill in your child's date of birth and your zip code, which pulls from information we have on file for your child. You will then be asked for a user name and password (for future logins please remember that the user name and password are case sensitive). All forms should be completed with your child's information, unless specifically asking for parent information. The first screen that appears is basic demographic information. Please verify this information, make necessary changes, and click "submit". (If you have more than one child, you will need to complete the above process for each of your children. You will then go the first child's account and click on "My Account" on the left of the page. Then across the top of the screen you will see a "Family" tab. From here you can link all your children to one account for easy access. Once all your children are linked, you can easily move from one child to the other by using the "change user" tab at the top of the screen.)
From the left - "My Patient Page" - allows you to see all the options such as appointment request, ask a billing question, ask front office, prescription refill, ask a nurse, request medical records, health forms, and online bill pay. No more than thirty (30) days prior to your yearly well check, you will need to go to "Health Forms" (this section contains both demographic information and health history information: and update your "Registration" and "Medical History" forms.
***If your child is coming for a yearly well check and you have updated your "Registration" and "Medical History" forms, you will need to complete your "Pre-visit Screening" forms. When you log out, you will automatically return to our website. Please click on the red "Online Services" door and choose the Pre-Visit Screening (Chadis) Icon.
Pre-Visit Screening (Chadis) Instructions
Click on the red "Online Services" door and choose the Pre-Visit Screening (Chadis) Icon. If you have never used Chadis before, click on New User down left hand column, Enter Invitation code: 9197865001. Complete Respondent Registration Process. If you already have a Chadis account click on Returning User to login and complete questionnaires for your child’s upcoming visit. Once you are logged in to Chadis, select the magnifying glass beside your child's name, then click on "Take Questionnaires", choose your doctor, choose your visit type, and you will see the questionnaires specific to your child's visit. Click "Begin" to complete the questionnaire. If you have any questions, please contact our office. We do ask that you fill these screening tools out at least two (2) days but no more than thirty (30) days prior to your child's visit.
